Québec
Ministry of Education Requirements
ALL students are required to have a permanent code (code permanent),
assigned by the Québec Ministry of Education. If you do not have a permanent code you
must complete and return the CODE PERMANENT DATA FORM with your
application so we can have a code created by the Québec Ministry of Education.

Transcripts
All applicants are required to declare and submit all academic records when applying for
admission. Applicants who fail to disclose an academic record at the time of application
may have their acceptance or registration cancelled. Academic records undeclared at the
point of application and subsequently submitted will not be used to upgrade a
students placement level.
An "official" transcript is a record of all secondary
or post-secondary education sent directly to the Office of the Registrar from each of your
previous educational institutions.
Cégep Applicants
You are required to submit, at the time of application, or as soon as possible thereafter,
a student copy of your cégep transcript. This transcript should show your record
to date including grades for all courses followed during the previous terms. It is your
responsibility to ensure that this copy is both accurate and complete. Failure to provide
this information could delay your admission to the University.
Official transcripts (in-progress and final) are obtained by the
Office of the Registrar from the Conférence des recteurs et des principaux des
universités du Québec (CRÉPUQ) as soon as they are available.
NOTE: Québec students must enter their code permanent on the application
form so that Concordia University may obtain an official cégep record of study from
CRÉPUQ. It is essential to include the code permanent to avoid delay in the
processing of the application.
Ontario Applicants
We recommend you submit a copy of your current high school record. In agreement with the
Ontario Universities Application Centre (OUAC), high school transcripts for
applicants are electronically transmitted to the universities. Please note that Concordia
will be able to access this information only if you have provided us with a valid OUAC
number and have also applied to universities in Ontario.
January Entry: The OUAC system does not transmit
transcripts during the Fall semester. Therefore, if you are applying for admission to the
Winter Term, you must arrange for the submission of an official transcript directly from
your high school.
Other Canadian
Provinces/United States
All secondary and post-secondary scholastic records must be submitted. It is your
responsibility to ensure that an official copy of your transcript is forwarded directly to
the Admissions Application Centre.
In an endeavour to accelerate the admissions process, and as a
convenience to you, the Office of the Registrar is now accepting official transcripts via
two methods: 1) official transcripts sent directly by the school to the Office of the
Registrar, and 2) "sealed-envelope" method. To utilize the
"sealed-envelope" method, you request that each previous educational institution
provides you with a copy of your official transcript, sealed in an envelope
which bears the name and address of the school, the institutional logo (if applicable), a
notice on the envelope stating that it contains official transcripts, and with a
signature/signature stamp/explanatory label placed across the seal of the envelope. If you
enclose such an envelope, unopened, along with your additional supplemental forms,
we will consider the transcript official.
If you are currently enrolled at an institution, you must also
submit the results of previous terms together with a list of courses in which you are
currently registered. An official transcript which includes the results of the final
semester must be forwarded as soon as possible. All documents must be originals or
appropriately certified copies. Uncertified copies are not acceptable. It should be noted
that scholastic records submitted by fax will not be considered as official transcripts.
Other Applicants
Students outside North America must arrange to have their schools mail an official
transcript directly to the Admissions Application Centre.
All transcripts should be in the original language. If you submit
certificates in a language other than English or French, you must also attach a translated
copy by a certified translator into either English or French. If the certificate does not
detail the subjects taken and the grades achieved, a certified statement of these from an
authorized official of the institution should also be submitted.

Citizenship Status
All applicants must provide documentary proof of their status in Canada. Applicants who
have been accepted by the University and who have not provided this proof of status will
automatically be charged the higher international tuition fees. To provide proof of
Canadian Citizen/Permanent Resident (Landed Immigrant) status you must submit a photocopy
to the Office of the Registrar of one of the following documents:
- Canadian Birth Certificate
- Canadian Baptismal Certificate showing place of birth
- Permanent Resident or Landed Immigrant citizenship papers
- Canadian Passport
The following documents are not acceptable:
- Driver's License
- Medicare Card
- Social Insurance Number card
