INSTRUCTIONS FOR COMPLETING THE 2010-11 FAFSA FORM

All students who wish to apply for any form of financial aid (including the Federal Stafford Loan) must first complete the 2010-2011 Free Application for Federal Student Aid (FAFSA) form.

Students who did not file a FAFSA form last year may complete the FAFSA at www.fafsa.ed.gov . You will not need a Personal Identification Number (PIN) if you are completing the FAFSA for the first time. However, you may request a PIN at www.pin.ed.gov should you desire to make future changes to your initial FAFSA data.

Graduate students who filed a FAFSA form last year may complete the 2010-2011 Renewal Form at www.fafsa.ed.gov. The U.S. Department of Education automatically mailed a PIN to all graduate students eligible to file a 2010-2011 renewal FAFSA. PINS were mailed to the permanent address listed on last year's FAFSA form. If you did not receive a PIN you may request a replacement PIN at www.pin.ed.gov . You will need a pin number to file the renewal FAFSA.

Please provide our office with a copy of the printed confirmation page, once you complete the FAFSA. This will serve as written verification that the FAFSA was filed by the established deadline date.

Failure to complete the required forms will delay the processing of your financial aid file. Students with incomplete files are not eligible to receive financial aid.

PLEASE NOTE: Students who wish to apply for institutional financial aid (i.e. MSM grant and scholarship aid) must complete the student and parent sections of the initial FAFSA or FAFSA renewal form (M.D. and M.P.H. students only). No parent signature is required.
